A 15-Year-Old Boy with Episodic Depression and Behavioral Problems for 1 Year

Holly was a 15-year-old boy who presented to his pediatrician because he had been depressed for about one year. He had no family history of depression or anxiety disorders. His mother described him as bright but socially isolated due to some behavioral issues in school that began when he was 10 years old. At the time Holly first came to the doctor’s office, he also complained of some difficulty falling asleep at night despite sleeping through the night previously without any problems. During his presentation at age 13, Holly referred to himself as “the loner kid” who played video games all day long and showed little interest in sports activities nor did he have many friends outside of school that were close enough for him to confide in or seek help from on an emotional level. On examination there were no significant findings except for mild submandibular lymphadenopathy along with normal thyroid function testing results; however, overall physical examination revealed moderate weight loss since last seen by doctor several months before admission (weight loss of 5 lb) which is not uncommon with patients who present late after menses has completed its cycle or onset has occurred within 2 weeks prior
